By submitting this form, I give Adult & Teen Challenge PacWest permission to charge my account for the amount and frequency listed below on the dates indicated. I understand that all charges are non-refundable even in the instance that the student who requested the donation for their fundraising goal leaves after my donation payment has been processed. In most cases Adult & Teen Challenge PacWest is not allowed to notify me if the student who requested my donation towards their fundraising goal leaves the program. I understand that this agreement is effective until I submit a request to friendsandfamily@atcpacwest.com to change my contribution. I understand that the request for change must be initiated by me, the donor. I understand that cancellation requests require five (5) business days processing time and that any charges made before a cancellation request is processed are non-refundable.
